Home Health of Western Colorado is seeking an Occupational Therapist to join our team in Montrose, Colorado, delivering skilled rehabilitation services to clients in their homes. This part-time PRN position offers the flexibility to make a direct difference in patients' lives while building clinical expertise in community-based occupational therapy.
About the Role
As an Occupational Therapist with Home Health of Western Colorado, you'll provide in-home therapy services that help clients regain independence and improve their quality of life. You'll travel to patients' residences throughout Montrose and surrounding areas to conduct functional assessments, develop individualized treatment plans, and deliver evidence-based interventions tailored to each person's goals and living environment.
What You'll Do
- Evaluate clients' physical, cognitive, and psychosocial functioning to identify rehabilitation potential and barriers to independence
- Design and implement personalized occupational therapy treatment plans addressing activities of daily living, mobility, and participation goals
- Document progress, adjust interventions as needed, and communicate findings to the interdisciplinary care team
- Educate clients and caregivers on adaptive techniques, home safety modifications, and adaptive equipment recommendations
- Maintain compliance with regulatory standards and professional documentation requirements in home health settings
What We're Looking For
- Current, unrestricted Colorado Occupational Therapy license
- Experience in adult rehabilitation, medical, or community health settings (home health experience preferred)
- Strong clinical judgment, communication skills, and ability to work independently in clients' homes
- Commitment to patient-centered care and professional excellence in occupational therapy practice
