MDS Registered Nurse (RN) – PRN

Franciscan Ministries seeks a compassionate MDS Registered Nurse (RN) for a PRN position at Village at Victory Lakes in Lindenhurst, Illinois. As an MDS Nurse, you'll lead comprehensive resident assessments and develop individualized plans of care in accordance with state and federal regulatory standards, directly supporting the quality of life for our senior community.

Position Summary

The MDS Registered Nurse plays a pivotal role in our senior care community by conducting thorough resident assessments, documenting health information, and creating evidence-based care plans that align with clinical best practices and regulatory requirements. Working under the Director of Nursing, you'll ensure every resident receives thoughtful, coordinated care tailored to their unique needs.

Day-to-Day Responsibilities

• Conduct comprehensive Minimum Data Set (MDS) assessments and reassessments per regulatory timelines
• Develop and update individualized resident care plans based on assessment findings
• Document resident medical history, current conditions, and care needs in compliance with state and federal guidelines
• Collaborate with the interdisciplinary care team—physicians, therapists, social workers, and direct care staff—to optimize resident outcomes
• Communicate assessment findings and care recommendations clearly to nursing leadership
• Monitor resident health status and adjust care plans as clinical conditions change
• Ensure all documentation meets regulatory compliance standards and is completed accurately and timely

Qualifications

• Current Registered Nurse (RN) license in good standing in Illinois
• Experience with Minimum Data Set (MDS) assessments and long-term care environments preferred
• Strong knowledge of Medicare, Medicaid, and state regulatory requirements
• Excellent clinical assessment and documentation skills
• Ability to work collaboratively across disciplines
• Professional communication skills and attention to detail
