FWS is hiring a Licensed Practical Nurse (LPN) for full-time care delivery at our skilled nursing facility in Warren, Marshall County, Minnesota. We're seeking a compassionate healthcare professional ready to start immediately and make a meaningful difference in our residents' lives.

About the Role

In this full-time long-term care nursing position, you'll provide essential direct patient care on a days schedule featuring 8-hour shifts during the week and 12-hour shifts on weekends. Every-other-weekend and every-other-holiday rotation provides predictable time off while ensuring strong continuity for residents.

What You'll Do

• Deliver hands-on personal care including bathing, grooming, toileting, and dressing assistance
• Monitor vital signs and report clinical changes to the nursing team
• Support wound care, catheter management, and nursing interventions under RN direction
• Respond promptly to resident requests and maintain comfort and safety
• Document care activities and communicate effectively with the interdisciplinary care team
• Advocate for resident dignity, independence, and quality of life in long-term care settings

What We're Looking For

• Current Minnesota LPN/LVN licensure in good standing
• Experience in long-term care, skilled nursing, or geriatric healthcare
• Strong clinical assessment and patient safety skills
• Excellent communication and teamwork across all shifts
• Genuine compassion for older adults and commitment to person-centered care
• Ability to work rotating days including weekends and flexible scheduling
