Facilitator I – Community Wraparound

Sycamores is hiring a Facilitator I – Community Wraparound in Los Angeles, Los Angeles County. This is a full-time position working directly with at-risk youth and families to coordinate mental health services and build sustainable support networks.

What This Role Involves

As a Community Wraparound Facilitator, you'll serve as a connector between families and the mental health resources they need most. You'll organize both formal clinical services and informal community supports, helping low-income and high-need families access care, navigate systems, and build lasting stability. This role sits at the heart of Sycamores' mission to serve youth experiencing significant challenges.

Your Responsibilities

• Coordinate and organize formal mental health services and informal community supports tailored to each family's unique needs
• Partner directly with families to develop, refine, and implement comprehensive wraparound service plans
• Facilitate communication between clinical providers, case managers, schools, and community partners on behalf of families
• Identify gaps in services and connect families with additional resources to strengthen their support networks
• Document progress, maintain accurate records, and report on family outcomes and service coordination efforts
• Attend team meetings and participate in collaborative care planning with clinical and community staff

What We're Looking For

• High school diploma or equivalent
• Strong written and verbal communication skills, with ability to explain complex services in clear language
• Ability to build rapport and work effectively with families, particularly from low-income and underserved communities
• Organizational skills and ability to manage multiple concurrent families and coordination tasks
• Commitment to trauma-informed and culturally humble practice
• Reliable transportation
