Director of Social Services

Mountain View Rehabilitation and Senior Living Center in Sunnyside, Northumberland County is hiring a full-time Director of Social Services. In this leadership role, you'll develop and oversee comprehensive social services programs that enhance the quality of life for our residents while coordinating care with clinical and administrative teams.

About the Role

As Director of Social Services, you'll lead a vital department committed to supporting the psychosocial well-being of residents in our rehabilitation and skilled nursing environment. This is a full-time management position reporting to senior leadership, where you'll shape how our community addresses emotional, social, and practical needs alongside clinical care.

What You'll Do

• Develop, implement, and evaluate social services programs aligned with facility policies and regulatory standards
• Supervise and mentor social services staff, ensuring quality assessments, care coordination, and resident advocacy
• Conduct comprehensive psychosocial assessments and care planning to support resident outcomes
• Coordinate with nursing, therapy, medical, and administrative departments to integrate social services into overall care delivery
• Manage discharge planning, including housing, community resources, and continuity of care arrangements
• Build relationships with community agencies, family members, and external service providers
• Ensure compliance with state and federal regulations governing social services in long-term care and rehabilitation settings
• Document social services activities and outcomes according to regulatory and facility documentation standards
• Participate in resident care conferences, quality improvement initiatives, and facility governance

What We're Looking For

• Master's degree in Social Work (MSW) or related field from an accredited program
• Current state licensure (LCSW, LMSW, or equivalent) required
• Minimum 3–5 years of experience in social services, preferably in senior living, rehabilitation, or long-term care
• Demonstrated leadership experience managing teams and programs
• Knowledge of aging services, rehabilitation practices, and regulations affecting senior care facilities
• Strong communication and interpersonal skills with residents, families, staff, and community partners
• Proficiency with electronic health records and standard office technology
• Ability to advocate effectively while maintaining professional boundaries and facility policies
