What This Role Involves
As a Case Manager, you'll be the bridge between clients and vital services, coordinating care and advocating for people within our behavioral health and justice systems. This position requires direct client contact, relationship-building skills, and a commitment to person-centered support. You'll work full-time in our Kew Gardens, Queens office as part of a mission-driven team.
What You'll Do
- Assess client needs and develop individualized care plans
- Connect clients with community resources, mental health services, and social benefits
- Monitor client progress and adjust support strategies as needed
- Maintain detailed case documentation and communication records
- Collaborate with healthcare providers, justice system partners, and family members
- Provide crisis support and intervention when required
- Advocate for client rights within behavioral health and criminal justice systems
Who We're Looking For
- High school diploma or GED (Bachelor's degree in social work, counseling, or related field preferred)
- Bilingual ability (preferred)
- Demonstrated experience working with behavioral health or justice-involved populations
- Strong communication and interpersonal skills
- Ability to manage multiple cases and priorities
- Commitment to cultural humility and trauma-informed practice
- Valid driver's license and reliable transportation (as needed)
