About the Role

This case manager position uses a comprehensive, individualized approach to help clients break free from homelessness. You'll support people at every stage—from street-level outreach through interim housing, transitional housing, and into permanent housing. Your role is to connect clients with essential services, coordinate their care, and help them achieve both immediate and long-term goals through personalized service planning and ongoing advocacy.

What You'll Do

  • Conduct thorough intake assessments and work with clients to create customized Individualized Service Plans (ISP) addressing housing, income, health, mental health, substance use, financial stability, employment, and social support
  • Connect clients with referrals and resources across healthcare, social services, and community support systems
  • Administer evidence-based assessments like the VI-SPDAT to guide service delivery
  • Review progress milestones at 90 days and adjust plans based on client needs and outcomes
  • Navigate the coordinated entry system to ensure clients access appropriate services and housing
  • Build relationships with colleagues, service partners, and community stakeholders to strengthen the support network

What We're Looking For

  • Strong analytical and attention-to-detail skills to track data, identify trends, and produce thorough assessments
  • Excellent written and verbal communication with diverse individuals and groups
  • Ability to work collaboratively across teams and with clients from varied backgrounds
  • Creative problem-solving and initiative to develop practical solutions in complex situations
  • Sound judgment, integrity, and professional boundaries in sensitive circumstances
  • Valid California driver's license, reliable transportation, and proof of insurance (driving is essential to this role)
  • Ability to pass background screening, drug test, tuberculosis screening, and obtain CPR/First Aid certification
  • Work authorization in the United States

Preferred: Two years of experience in case management, healthcare, homeless services, or work with vulnerable populations. All education and experience levels are welcome to apply.
