What This Role Involves
The Case Manager is responsible for the coordination and management of comprehensive patient care services to improve health outcomes, enhance satisfaction, and reduce unnecessary healthcare utilization. As part of the Enhanced Care Management (ECM) program, this role focuses on high-need populations with complex and chronic conditions.
