About the Role
In this supervisory position, you'll lead a team of six CADI Case Managers providing essential case management services to individuals enrolled in the Hennepin County Waiver program. Your leadership ensures clients receive coordinated, high-quality support tailored to their unique needs—aligning with person-centered principles and organizational excellence standards.
What You'll Do
- Supervise, mentor, and develop a team of six Case Managers delivering waiver services
- Ensure compliance with Hennepin County, state, and Peopleorporated Mental Health Services policies and procedures
- Monitor service quality and client outcomes across your team's caseload
- Coordinate with county agencies, providers, and community partners to support integrated care
- Review case documentation, treatment plans, and progress notes for accuracy and completeness
- Address performance issues and support continuous professional growth
- Participate in program planning and quality improvement initiatives
- Maintain accurate records and reporting requirements for waiver program compliance
What We're Looking For
- Proven supervisory or leadership experience in case management, mental health, or social services
- Knowledge of Hennepin County Waiver programs or similar community-based disability services
- Strong understanding of person-centered service delivery and recovery principles
- Excellent organizational, communication, and interpersonal skills
- Ability to manage multiple priorities and support a diverse team
- Familiarity with documentation, compliance, and quality assurance processes
