About the Role

As a Physical Therapist Assistant with Phoenix Home Care and Hospice, you'll work directly with patients to implement therapeutic interventions prescribed by our licensed Physical Therapists. In our home-based care environment, you'll help patients recover from illness and injury, manage chronic conditions, and achieve meaningful functional goals. This full-time position offers direct patient contact, clinical autonomy within your scope of practice, and the profound satisfaction of supporting patients during their rehabilitation journey.

Core Duties

• Perform prescribed therapeutic exercises, stretching, and manual techniques to restore patient strength and mobility
• Assist patients with functional mobility training and activities of daily living within treatment protocols
• Monitor and document patient responses to therapy, progress toward goals, and any clinical changes
• Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention strategies
• Maintain regular communication with supervising Physical Therapists regarding patient status and treatment effectiveness
• Ensure clean, safe therapy environments and maintain accurate clinical documentation

Requirements

• Current Physical Therapist Assistant license or certification for the state of Missouri
• High school diploma or equivalent; associate degree in Physical Therapy preferred
• Valid CPR and Basic Life Support certification
• Ability to assess patient capabilities and adapt treatment strategies based on individual needs
• Excellent communication and interpersonal skills with diverse patient and family populations
• Reliable transportation and ability to travel to patient homes within service areas
