Core Duties Perform prescribed therapeutic exercises, stretching, and manual techniques to restore patient strength and mobility Assist patients with functional mobility training and activities of daily living within treatment protocols Monitor and document patient responses to therapy, progress toward goals, and any clinical changes Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention strategies Maintain regular communication with supervising Physical Therapists regarding patient status and treatment effectiveness Ensure clean, safe therapy environments and maintain accurate clinical documentation Requirements Current Physical Therapist Assistant license or certification for the state of Missouri High school diploma or equivalent; associate degree in Physical Therapy preferred Valid CPR and Basic Life Support certification Ability to assess patient capabilities and adapt treatment strategies based on individual needs Excellent communication and interpersonal skills with diverse patient and family populations Reliable transportation and ability to travel to patient homes within service areas Working at Phoenix Home Care and Hospice Phoenix Home Care and Hospice is dedicated to providing compassionate, patient-centered care throughout the communities we serve.
