Position Summary

As a Physical Therapist with Phoenix Home Care and Hospice in Pittsburg, you'll provide skilled physical therapy services to patients receiving home care and hospice support. You'll work directly with patients and interdisciplinary teams to develop and implement evidence-based treatment plans, helping individuals maintain function, manage pain, and achieve meaningful rehabilitation goals within the comfort of their homes.

Day-to-Day Responsibilities

  • Conduct comprehensive physical therapy assessments and evaluations for patients referred to home care and hospice services
  • Develop, implement, and modify individualized treatment plans based on patient needs, medical history, and functional goals
  • Provide direct physical therapy interventions, including therapeutic exercises, manual therapy, gait training, and mobility assistance
  • Document patient progress, treatment outcomes, and clinical observations in accordance with medical and regulatory standards
  • Communicate and collaborate with physicians, nurses, social workers, and family members to ensure coordinated, patient-centered care
  • Educate patients and caregivers on exercises, safety strategies, and techniques to support ongoing independence at home
  • Monitor patient responses to treatment and adjust therapeutic approaches as clinically indicated

Requirements

  • Valid Physical Therapist license in the State of Kansas
  • Doctor of Physical Therapy degree (DPT) from an accredited program
  • Demonstrated experience in home care, hospice, or acute care settings preferred
  • Strong clinical assessment and documentation skills
  • Excellent interpersonal and communication abilities
  • Ability to work independently and as part of an interdisciplinary care team
  • Valid driver's license and reliable transportation for in-home patient visits
