Physical Therapist (PT) – PRN

Fira-health is hiring a Physical Therapist (PT) to deliver in-home rehabilitation services throughout Oakland County on a flexible, part-time basis, earning $75 per productivity point plus a $1,000 sign-on bonus after 90 days and referral bonuses. You'll manage patient care from assessment through discharge and help establish care standards for our growing home health practice in the Detroit area.

What This Role Involves

• Providing skilled physical therapy services to patients in their homes across Oakland County
• Conducting thorough evaluations to assess functional limitations, mobility deficits, and therapy needs
• Directing patient care including treatment planning, progress monitoring, and discharge decisions
• Working independently in the field with clinical autonomy and support from the care team
• Collaborating with physicians, nurses, and other clinicians to optimize patient outcomes

What You'll Do

• Complete in-home physical therapy assessments and deliver skilled therapeutic interventions
• Develop individualized treatment plans addressing balance, gait, strength training, transfers, and functional independence
• Set objective clinical goals and regularly measure patient progress
• Modify therapy approaches based on patient response and clinical status changes
• Supervise physical therapy assistants where needed
• Teach patients and family members practical home exercise programs and safety strategies
• Communicate clinical concerns and recommendations to physicians and the broader care team
• Maintain clear, timely documentation in Homecare Homebase electronic health records

Requirements

• Current, valid Michigan physical therapy license
• Demonstrated ability to work independently and think clinically in home-based settings
• Strong communication and documentation skills
• Valid driver's license with reliable transportation for frequent travel throughout assigned service area
• Physical capability to safely handle and transport therapy equipment
• Competency in core home health physical therapy:
  • Balance and gait training with emphasis on fall prevention
  • Safe transfer instruction and assistive device recommendations
  • Therapeutic strengthening and functional mobility training
  • Home environment assessment and adaptive strategy education
  • Patient and caregiver coaching on practical home programs
• Preferred experience: prior home health background, familiarity with Homecare Homebase or similar EMR platforms, PTA supervision experience, or work with complex post-acute populations
