Position Summary

This is a field-based, part-time role where you'll work on your own schedule, picking up visits that align with your availability across weekdays and weekends. As an OT at Fira-health, you'll conduct in-home evaluations, deliver skilled occupational therapy, and coordinate closely with nurses and physicians to achieve strong patient outcomes and build lasting relationships with referral partners in the healthcare community.

Day-to-Day Responsibilities

  • Perform comprehensive in-home occupational therapy evaluations and deliver skilled treatment visits tailored to each patient's needs.

  • Develop and implement individualized plans of care focused on activities of daily living (ADLs), instrumental activities of daily living (IADLs), home safety, and fall prevention.

  • Assess for, recommend, and train patients and caregivers on adaptive equipment, assistive devices, and home modifications to support functional independence.

  • Collaborate with the nursing team and interdisciplinary care staff when patient conditions change or new needs emerge.

  • Document evaluations, progress, and clinical decisions promptly and thoroughly in Homecare Homebase (HCHB).

What You Bring

  • Active, unrestricted occupational therapy license from the State of Michigan.

  • Strong experience or proven comfort working independently in home health settings with sound clinical judgment.

  • Ability to recognize and appropriately escalate changes in patient condition to physicians and nurses with clear documentation of your clinical reasoning.

  • Excellent verbal and written communication skills; ability to document in a way that's thorough, accurate, and defensible.

  • Reliable execution of treatment plans, visit frequencies, and patient goals with demonstrated follow-through.

  • Valid driver's license, current auto insurance, reliable transportation, and comfort traveling daily across your assigned service area to visit patients at home.

  • Physical ability to safely carry and transport basic occupational therapy equipment.

  • Solid foundation in core home health occupational therapy practice: ADL/IADL training, home safety assessment, adaptive equipment selection, and family/caregiver education.
