Care Facilitator

ChenMed is seeking a full-time Care Facilitator to join our De Kalb, Kershaw County medical practice. As a critical member of our primary care team, you'll deliver exceptional front-line patient support, drive operational efficiency, and help us continue building a healthcare experience that prioritizes both patient outcomes and team satisfaction in rural South Carolina.

About the Role

The Care Facilitator is the first point of contact for our patients, welcoming them to our clinic and ensuring a smooth, supportive experience from arrival through their appointment. You'll work alongside our clinical team to keep our practice running smoothly while building genuine connections with each patient you serve.

What You'll Do

• Check in patients, gather demographic and insurance information, and verify appointment details
• Schedule appointments and manage patient flow throughout the day
• Answer patient calls and address routine inquiries with professionalism and compassion
• Assist with vital signs and basic patient intake procedures as directed by clinical staff
• Communicate with insurance companies and external healthcare providers to support continuity of care
• Maintain organized medical records and ensure patient confidentiality and HIPAA compliance
• Support clinical team members with administrative and patient-care tasks
• Identify patient needs and connect them with community resources and health education materials

What We're Looking For

• High school diploma or equivalent; medical administrative experience or certification preferred
• Strong interpersonal skills and a genuine ability to connect with patients from diverse backgrounds
• Excellent communication skills, both verbal and written
• Organized, detail-oriented, and able to manage multiple priorities
• Comfortable learning and using electronic health records (EHR) systems
• Reliable, punctual, and committed to delivering quality patient care
• Ability to work as part of a collaborative team in a fast-paced primary care setting
