Hartford HealthCare at Home seeks a Hospice Weekend RN (Registered Nurse and Visiting Nurse) for full-time weekend work in the New Britain, Hartford County area. This position offers meaningful work with patients and families during critical end-of-life care moments.

About the Role

As a weekend float registered nurse in our hospice program, you'll provide compassionate, skilled nursing care to patients in their homes and community health settings across our west hospice regions. This visiting nurse role ensures continuity of care when patients and families need support most, delivering high-quality, outcome-focused care that honors patient dignity and comfort during end-of-life transitions.

What You'll Do

• Provide skilled nursing assessments and interventions for hospice patients in home and community settings
• Review patient referrals and establish initial contact with clients and their families
• Develop and implement individualized care plans focused on comfort and quality of life
• Coordinate with the interdisciplinary hospice team to ensure comprehensive end-of-life care
• Monitor patient symptoms and communicate changes to the care team and family members
• Document clinical findings and patient progress in compliance with regulatory standards
• Support families through the hospice experience with education and compassionate presence

What We're Looking For

• Current Connecticut RN license (Registered Nurse) in good standing
• Experience or demonstrated interest in hospice and palliative care nursing
• Strong communication skills and ability to build trust with patients and families
• Clinical judgment and ability to work independently in home settings
• Flexibility to work weekend schedules and respond to patient and family needs
• Commitment to compassionate, outcome-based care delivery
