About the Role
As a Social Needs Navigator supporting our Advancing Whole Health initiative, you will serve as a trusted link between patients and the resources that improve their daily lives. Working on-site in Forest Hill, Richmond, you will coordinate closely with YMCA colleagues and each patient's broader care team to address the social factors that shape long-term health outcomes within our chronic disease health programs.
Key Responsibilities
- Accept and process incoming referrals into the program in a timely, organized manner
- Manage an assigned caseload, tracking each patient's progress and follow-up needs
- Build and maintain individualized care plans that respond to patients' social and health-related needs
- Partner with internal YMCA team members to deliver coordinated, patient-centered support
- Collaborate with external members of each patient's care team to ensure continuity of care
- Encourage and motivate patients as they work toward healthier, more stable lives
Qualifications
- A positive, encouraging, and dedicated approach to supporting people in need
- Strong organizational skills to balance referrals, caseloads, and follow-ups effectively
- Comfort collaborating across internal teams and external care providers
- A genuine commitment to community health and the mission of the YMCA
- Ability to work on-site in Forest Hill, Richmond on a full-time basis
