Oak Street Health is hiring a New Graduate Nurse Practitioner Fellow for our Albuquerque primary care clinic. This full-time fellowship program provides recent NP graduates with mentored clinical experience and professional development to establish a strong foundation in independent primary care practice.

About the Role

As a New Graduate Nurse Practitioner Fellow at Oak Street Health, you'll deliver evidence-based primary care to our patients while developing clinical competence through structured mentorship. This fellowship bridges the transition from graduate education to independent practice in a supportive, team-based environment where learning and growth are prioritized.

What You'll Do

• Provide direct patient care in a busy primary care setting serving diverse patient needs
• Conduct patient assessments, establish diagnoses, and develop treatment plans with clinical oversight and guidance
• Build therapeutic relationships with patients and coordinate care with interdisciplinary team members
• Participate in mentored clinical rounds and case discussions with experienced practitioners
• Contribute to quality improvement initiatives and safe, compassionate patient outcomes
• Engage in continuous learning and professional development throughout your fellowship year

What We're Looking For

• Master's degree in Nursing Practice or related advanced nursing field
• Active Nurse Practitioner licensure in New Mexico
• DEA registration and ability to obtain state credentialing
• Strong clinical reasoning, assessment, and interpersonal communication skills
• Dedication to evidence-based, patient-centered healthcare delivery
• Openness to learning and growth in a collaborative primary care environment
