About the Role
As a Mental Health Case Manager at Hope Unity Freedom Center (H.U.F. Center), you will work within a psychosocial day support program, walking alongside adults who live with primary mental health diagnoses, co-occurring conditions, and substance use challenges. Your steady presence helps participants build skills, regain stability, and move toward the independence they are striving for.
Key Responsibilities
- Coordinate individualized care for adults managing mental health and co-occurring disorders within a day support setting.
- Build trusting, supportive relationships that encourage participation, recovery, and personal growth.
