What You'll Do
- Deliver direct patient care including hygiene, comfort measures, and activities of daily living assistance
- Administer medications and treatments under clinical supervision
- Monitor vital signs and patient condition, reporting changes to the care team
- Perform wound care and ostomy care as assigned
- Maintain accurate patient documentation and medical records
- Collaborate with nursing staff, physicians, and interdisciplinary teams to support resident goals
- Foster a welcoming, dignity-focused environment for each resident
What We're Looking For
- Current LPN license in good standing
- Valid CPR certification
