In-Home Health – Nurse Practitioner or Physician Assistant (Part Time) – Saint Cloud, MN

CVS Health is seeking a Nurse Practitioner or Physician Assistant for in-home health services in the Saint Cloud, Minnesota area. Join our clinical team and deliver direct patient care in the comfort of patients' homes.

About the Role

In this full-time position, you'll provide evidence-based clinical care to patients in their home environment. You'll conduct patient evaluations, develop and implement treatment plans, and coordinate ongoing care with the healthcare team. This role combines clinical expertise with the meaningful impact of in-home medicine, allowing you to build lasting relationships with patients while supporting their health goals in a familiar setting.

What You'll Do

• Perform comprehensive health assessments and physical exams for patients in their homes
• Diagnose conditions and develop individualized treatment and care plans
• Prescribe medications and therapies in accordance with clinical protocols
• Manage chronic diseases and acute health conditions
• Educate patients and families on health management and medication compliance
• Document patient encounters and clinical findings in the electronic health record
• Collaborate with physicians, nurses, and other healthcare professionals
• Monitor patient progress and adjust treatment plans as needed

What We're Looking For

• Active NP or PA license in Minnesota
• Board certification (ANCC, AAPA, or equivalent)
• Minimum 2 years of clinical practice experience
• Strong assessment, diagnostic, and clinical decision-making skills
• Excellent communication and patient education abilities
• Ability to work independently and manage time effectively
• Proficiency with electronic medical records systems
• Current CPR certification
