Home Care Service Coordinator (Scheduler)

Ambercare is seeking a Home Care Service Coordinator (Scheduler) for a full-time position based in Alamogordo, Otero County. In this role, you'll be central to coordinating care services and ensuring that older adults and individuals with disabilities receive the support they need to remain safely and independently at home.

About the Role

As a Service Coordinator with Ambercare, you'll manage the scheduling of in-home caregivers while maintaining the highest standards of care quality. You'll serve as the liaison between clients, family members, caregivers, and our care management team, solving scheduling conflicts and ensuring continuity of care. Your organizational skills and compassion will directly impact our clients' ability to live with dignity in their own homes.

What You'll Do

• Schedule and coordinate caregiver assignments to match client needs and preferences
• Conduct or participate in in-home visits to assess care requirements and build client relationships
• Manage scheduling conflicts and adjust care plans responsively to ensure seamless service delivery
• Communicate with clients, families, and caregivers to confirm appointments and address concerns
• Monitor service quality and follow up on client satisfaction and caregiver performance
• Maintain accurate records and documentation related to client care and scheduling
• Respond to urgent scheduling needs and problem-solve on behalf of clients and our care team
