About the Role

As a Special Education Transition Services Project Facilitator, you will coordinate and support programs that prepare K-12 students with special needs for postsecondary education, employment, and independent living. Working closely with educators, families, and community partners, you will champion individualized transition planning that aligns with each student's goals and the standards set by Clark County School District.

Key Responsibilities

• Facilitate and oversee special education transition services across schools in Winchester, Clark County.
• Partner with teachers, counselors, and support staff to develop and implement student transition plans.
• Connect students and families with postsecondary, vocational, and community resources.
• Provide guidance, training, and coordination to ensure compliance with special education requirements.
