Day-to-Day Responsibilities

• Conduct in-home health coaching sessions with patients throughout Las Vegas, focusing on chronic disease management, medication understanding, nutrition, and lifestyle changes
• Create individualized health plans that align with each patient's circumstances, cultural values, and personal wellness goals
• Educate patients and families on disease prevention, health risk factors, and evidence-based wellness strategies
• Serve as a cultural liaison between patients and their healthcare providers, ensuring clear communication and respectful, inclusive care
• Document coaching interactions, track patient progress, and communicate outcomes to support coordinated care
• Travel throughout the Las Vegas region to meet patients in their homes

Qualifications

• Bilingual fluency in English and Spanish (speaking, reading, and writing)
• High school diploma, GED, or equivalent; health-related certification or degree strongly preferred
• Experience in health education, community health work, patient coaching, or similar healthcare support roles
• Excellent communication skills and demonstrated ability to work effectively with people from diverse backgrounds
• Valid driver's license and dependable personal transportation
• Strong organizational skills and ability to manage documentation requirements
