This role combines compassionate care with accountability—each day brings real opportunities to support people through meaningful, everyday activities and personal transitions.
Your Responsibilities
- Assist individuals with personal care, daily living activities, and community engagement based on individual care plans
- Monitor and document client status, progress, and any concerns; communicate regularly with clinical supervisors
- Maintain clean, safe, and welcoming environments in clients' homes and community spaces
- Support clients in building skills, pursuing goals, and participating in community life
- Follow established protocols for health, safety, and behavioral support
- Transport clients to appointments, activities, and community locations as needed
- Respond compassionately to clients' needs and alert supervisors to changes or concerns immediately
Who We're Looking For
- Reliable, patient, and genuinely committed to supporting vulnerable populations
- High school diploma, GED, or equivalent education
- Valid driver's license and clean driving record (required for some client locations)
- Ability to pass background screening and health requirements
- Strong communication skills and comfort working within clinical teams
- Physical capability to assist clients with mobility, transferring, and daily tasks
- Willingness to learn and follow comprehensive care plans and organizational protocols
- Prior experience as a caregiver, nursing assistant, or in direct support roles is a plus
