Case Management – Social Worker Discharge Planner

Mountain View Hospital seeks an experienced Social Worker to serve as a Discharge Planner, facilitating safe transitions and community care for our patients throughout Beachs Corner, Bonneville County. This full-time position bridges clinical care and community support, ensuring every patient leaves our facility with a comprehensive plan and the resources to thrive beyond hospitalization.

About the Role

The Social Worker Discharge Planner plays a vital role in our care coordination team, meeting patients and families at critical transitions. You'll assess individual and family circumstances, identify barriers to safe discharge, and develop individualized plans that connect people with the right services and supports—whether skilled nursing, therapy, in-home care, housing assistance, or financial resources.

What You'll Do

• Conduct comprehensive psychosocial and discharge planning assessments for all patients, documenting findings and recommendations in the medical record
• Collaborate with physicians, nurses, and clinical staff to design discharge plans that meet each patient's medical, social, and practical needs
• Connect patients and families with community resources, social services, benefit programs, and specialized care facilities
• Advocate for patients facing barriers such as housing instability, financial hardship, substance use concerns, or social isolation
• Provide counseling and emotional support to patients and families navigating health crises and life changes
• Follow up with patients post-discharge to ensure successful care transitions and engagement with recommended services

What We're Looking For

• Bachelor's degree in Social Work (BSW) or Master's degree in Social Work (MSW) from an accredited program
• Current licensure as a Licensed Clinical Social Worker (LCSW) or ability to obtain licensure in your state
• Strong knowledge of community resources, insurance coverage, and benefit programs in your region
• Excellent verbal and written communication skills and ability to document clearly in electronic health records
• Genuine compassion for vulnerable populations and commitment to patient advocacy
• Ability to work effectively within an interdisciplinary healthcare team and manage multiple cases simultaneously
