PATH (People Assisting the Homeless) is hiring a Case Manager for our Time Limited Subsidy (TLS) program at the RHC site in Los Angeles County. In this full-time position, you'll provide intensive case management and supportive services to individuals and families experiencing homelessness, connecting them to housing and the resources necessary for sustained housing stability.
About the Role
As a Case Manager with PATH's TLS program in Los Angeles, you'll work Monday through Friday, 8:00 a.m. to 4:30 p.m., delivering evidence-based supportive services to clients across all phases of their homelessness experience—from street outreach through permanent housing. Your work directly supports PATH's mission of guiding approximately 23 individuals per week toward independent, stable living situations. You'll combine deep empathy with practical problem-solving, using motivational interviewing and trauma-informed practices to help clients navigate systems and achieve their housing goals.
What You'll Do
- Conduct intake interviews and strengths-based assessments to identify client needs, preferences, and barriers to housing stability
- Develop individualized service plans with clients addressing housing goals, income, health, mental health, substance-use needs, and social supports
- Administer standardized assessment tools including the VI-SPDAT to inform service planning and resource allocation
- Connect clients with community resources across healthcare, benefits, employment, mental health, and housing services
- Document case progress thoroughly and maintain complete, confidential client files per HIPAA and PATH policies
- Build sustained relationships with clients and systems partners to enhance engagement and retention
- Provide crisis intervention and safety assessment for clients in acute distress
- Conduct regular 90-day reviews and adjust support strategies based on client progress and evolving needs
What We're Looking For
- Analytical capability and precision in data interpretation and documentation
- Collaborative style; ability to work effectively with diverse colleagues, participants, and community organizations
- Clear communication—both written and verbal—with varied audiences
- Proactive, creative approach to solving client challenges
- Strong ethics, integrity, and commitment to professional boundaries
- Preferred: 2 years' experience in healthcare, case management, homeless services, or work with vulnerable populations
Mandatory Requirements
- U.S. work authorization
- Valid driver's license, reliable vehicle, proof of insurance, and vehicle ownership (driving is essential to this role)
- CPR and First Aid certification, or willingness and ability to obtain
- Pass background screening, tuberculosis testing, and drug screening

Compensation and Benefits
