About the Role
This Case Manager position is based in Long Beach, CA 90805 and operates Monday through Friday from 8:00 AM to 4:30 PM as a non-exempt role. As part of PATH's Time Limited Subsidy initiative, you'll employ a comprehensive "whatever it takes" approach to connect clients with housing and the services they need to thrive. Your work spans the full spectrum of homelessness—from street outreach through interim housing, transitional housing, and permanent housing placements—ensuring each person receives the individualized support necessary to break the cycle of homelessness.
What You'll Do
- Conduct intake assessments and collaborate with clients to develop Individualized Service Plans addressing housing goals, income, health, and personal objectives
- Link clients to healthcare, mental health, substance use, financial, and vocational resources through referrals and direct support
- Administer program assessments such as VI-SPDAT to identify need levels and appropriate service pathways
- Review client progress at 90-day intervals and adjust service plans based on outcomes and changing needs
- Maintain detailed, timely case documentation and participant files in compliance with HIPAA and agency policies
- Build therapeutic relationships grounded in motivational interviewing and trauma-informed care principles
- Provide crisis intervention and risk assessment to enhance client safety and independent problem-solving capacity
- Stay current on community resources and coordinate wrap-around services that address barriers to housing stability
What We're Looking For

- Driving Capability: Possess a valid driver's license, reliable transportation, proof of insurance and vehicle ownership, and ability to qualify for PATH insurance coverage (driving is an essential job function)
Preferred: Two years of experience in healthcare, case management, homeless services, or work with vulnerable populations. Minimum: All education and experience levels welcome. Required: Employment authorization, CPR/First Aid certification (or willingness to obtain), TB screening, background check clearance, and drug test completion prior to hire.
