About the Role

As a Peer Case Manager with the Outreach team at PATH's North Long Beach office, you'll engage in frontline services designed to break cycles of homelessness through personalized support and resourceful problem-solving. This position requires reliable transportation and a valid driver's license, as you'll work both in-office and in the field throughout Los Angeles County.

What You'll Do

  • Conduct comprehensive intake assessments and develop individualized service plans tailored to each participant's housing stability, income, medical, mental health, and vocational goals
  • Connect clients with community resources, referrals, and support services that advance their progress toward housing security
  • Administer evidence-based assessment tools such as the VI-SPDAT to inform service delivery
  • Perform structured reviews at 90-day intervals to evaluate progress and adjust support strategies
  • Maintain accurate, confidential case documentation in compliance with HIPAA and organizational standards

What We're Looking For

  • Strong analytical abilities and meticulous attention to detail in managing participant records and identifying service opportunities
  • Effective communication skills—both written and verbal—when working with diverse populations and multidisciplinary teams
  • Demonstrated ability to think creatively, solve problems independently, and take initiative without constant direction
  • Genuine commitment to ethical practice, professional boundaries, and maintaining participant dignity
  • Capacity to work collaboratively across departments and with external partners toward shared outcomes
  • Preferred: Two years of direct experience in case management, healthcare, homeless services, or support for vulnerable populations
  • Valued: Lived experience with recovery from homelessness, mental health challenges, or substance use—bringing personal insight to client interactions
  • Required: Valid driver's license, reliable personal vehicle, proof of insurance, and willingness to obtain CPR/First Aid certification
