Case Manager – Villas on 4th (JR 5682)

PATH (People Assisting the Homeless) is hiring a Case Manager for its Villas on 4th program in San Jose, CA to support individuals experiencing homelessness in achieving stable housing and self-sufficiency. This full-time, on-site position in Santa Clara County lets you deliver direct, personalized advocacy and resources to people during critical transitions.

About the Role

As Case Manager, you will serve clients across the full spectrum of homelessness—from street-level outreach through permanent housing placement and stabilization. You'll combine clinical case management with trauma-informed, strength-based methods that respect client choice and build toward lasting independence. Your approach emphasizes comprehensive problem-solving: whatever obstacles arise, you help clients navigate systems, access services, and secure safe housing.

What You'll Do

• Assess client needs comprehensively and develop individualized service plans targeting housing, income growth, health, and wellness
• Connect clients with healthcare providers, mental health treatment, substance use recovery resources, benefits programs, and job services
• Conduct standardized assessments including VI-SPDAT and facilitate 90-day check-ins to review progress and adjust support
• Maintain complete, confidential case files with timely progress notes per HIPAA and program policy
• Provide crisis response and safety planning, helping clients develop coping tools and resilience
• Apply motivational interviewing and trauma-responsive practices to build trusting, therapeutic relationships with clients
• Coordinate with team members and community organizations to deliver integrated, wraparound services
• Conduct field-based meetings and home visits as an essential part of engagement

What We're Looking For

• Excellent communicator—clear writing, effective listening, and genuine connection across diverse groups
• Organized and detail-focused; you track complex information, spot important patterns, and maintain accurate records
• Strong collaborator who builds productive relationships with colleagues, partners, and the people you serve
• Resourceful self-starter; you identify gaps, design solutions, and follow through on your own initiative
• Person of integrity with clear professional boundaries who earns trust through consistency and honesty
• Current, valid driver's license and reliable personal transportation with proof of insurance (driving is essential)
• CPR and First Aid certification, or commitment to obtain before hire
• Willingness to complete tuberculosis screening, background check, and drug test
• Authorized to work in the United States
• Preferred: 2+ years in case management, homelessness services, healthcare, or similar fields
