About the Role

As Case Manager, you will serve clients across the full spectrum of homelessness—from street-level outreach through permanent housing placement and stabilization. Your approach emphasizes comprehensive problem-solving: whatever obstacles arise, you help clients navigate systems, access services, and secure safe housing.

What You'll Do

  • Assess client needs comprehensively and develop individualized service plans targeting housing, income growth, health, and wellness
  • Connect clients with healthcare providers, mental health treatment, substance use recovery resources, benefits programs, and job services
  • Conduct standardized assessments including VI-SPDAT and facilitate 90-day check-ins to review progress and adjust support
  • Maintain complete, confidential case files with timely progress notes per HIPAA and program policy
  • Provide crisis response and safety planning, helping clients develop coping tools and resilience
  • Apply motivational interviewing and trauma-responsive practices to build trusting, therapeutic relationships with clients
  • Coordinate with team members and community organizations to deliver integrated, wraparound services
  • Conduct field-based meetings and home visits as an essential part of engagement

What We're Looking For

  • Excellent communicator—clear writing, effective listening, and genuine connection across diverse groups
  • Organized and detail-focused; you track complex information, spot important patterns, and maintain accurate records
  • Strong collaborator who builds productive relationships with colleagues, partners, and the people you serve
  • Resourceful self-starter; you identify gaps, design solutions, and follow through on your own initiative
  • Person of integrity with clear professional boundaries who earns trust through consistency and honesty
  • Current, valid driver's license and reliable personal transportation with proof of insurance (driving is essential)
  • CPR and First Aid certification, or commitment to obtain before hire
  • Willingness to complete tuberculosis screening, background check, and drug test
  • Authorized to work in the United States
  • Preferred: 2+ years in case management, homelessness services, healthcare, or similar fields
