About the Role
The Case Manager position centers on helping clients break free from homelessness through comprehensive, individualized support. You'll work across all stages of the housing continuum—from street outreach through interim housing to permanent placements—providing the advocacy, connections, and skills coaching that enable lasting change. You'll maintain a standard Monday-Friday, 8:30 a.m. to 5:00 p.m. schedule and use reliable transportation to conduct in-person client work throughout the service area.
What You'll Do
- Conduct thorough intake assessments and partner with clients to create personalized service plans addressing housing, income stability, health, mental wellness, and social support goals
- Complete validated screening tools (including the VI-SPDAT) to identify strengths and priority needs
- Connect clients with healthcare, mental health, substance use treatment, vocational services, and other community resources; follow up to ensure they successfully access and benefit from these supports
- Perform regular check-ins and 90-day progress reviews to evaluate goal achievement and adjust service plans as needed
- Intervene during crises and assess risk, drawing on trauma-informed practices and motivational interviewing to help clients build coping skills and regain agency
- Maintain accurate, confidential case documentation that meets program and funder requirements while fully honoring HIPAA and privacy protections
- Build strong, professional relationships with clients and partner organizations to enhance engagement and improve retention in housing and services

- Reliable transportation: a valid driver's license, current insurance, and a well-maintained vehicle are essential to this role
- Willingness to complete pre-employment screening: a TB test, background check, and drug test are required. CPR/First Aid certification (or commitment to obtain it) is mandatory

- Two or more years working in healthcare, case management, homeless services, substance use treatment, mental health, or similar fields that serve vulnerable populations
