Together for Youth seeks a Bilingual Case Manager for our Van Cott office in Bronx. This full-time position is central to delivering coordinated counseling, educational support, and case management services to youth participants in our programs. You will combine planning, coordination, and monitoring expertise to ensure young people receive comprehensive interventions while serving as the primary liaison between our agency and partner organizations.
About the Role
As Bilingual Case Manager, you will manage all dimensions of participant case coordination—from intake and assessment through program completion. Your fluency in both English and Spanish positions you to build trust, communicate clearly with participants and families, and remove barriers to care. You'll work directly with youth to design individualized support pathways and hold responsibility for continuous monitoring and program quality assurance.
What You'll Do
- Develop and implement comprehensive case plans aligned with participant goals and agency interventions
- Coordinate counseling, education, and support services across internal and external partners
- Monitor participant progress, adjust plans as needed, and document outcomes
- Represent Together for Youth in recruiting program participants and evaluating referral requests
- Serve as the primary point of contact and advocate for participants and families
- Communicate with schools, social services, and community organizations to facilitate coordinated care
- Maintain detailed, accurate case records and contribute to agency reporting and compliance requirements
What We're Looking For
- Bachelor's degree required
- Bilingual fluency in English and Spanish (verbal and written)
- Demonstrated experience in case management, social services, counseling coordination, or youth development
- Strong organizational skills and ability to manage multiple participant caseloads
- Excellent written and verbal communication with diverse stakeholders
- Commitment to working with young people and supporting their growth
- Ability to work independently and as part of a collaborative team
