Day-to-Day Responsibilities

Requirements
- Bachelor's degree in Social Work or a related field
- Licensed as a Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Experience working with diverse populations and managing caseloads effectively
- Strong communication, organizational, and problem-solving skills
- Prior experience in home care or post-acute care settings preferred
