Role Overview

We are seeking a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in Fair Oaks, Texas. As part of the Optum family of businesses through WellMed, we are dedicated to providing high-quality healthcare services and support for patients in need.

Your Responsibilities

• Assess patient needs and provide appropriate care based on medical orders
• Administer medications as prescribed by a physician
• Perform basic nursing tasks such as wound care, catheter management, and vital sign monitoring
• Provide education to patients and families regarding health conditions and self-care practices
• Document patient care activities accurately in the medical record
• Communicate with interdisciplinary team members to ensure coordinated care for patients

What You Bring

• A valid LVN or LPN license from Texas
• Current CPR certification
• Minimum of 1 year experience in home health nursing preferred but not required
• Strong communication and interpersonal skills
• Patient-centered approach to care
• Ability to work independently and as part of a team
