About the Role

This full-time Physical Therapist / Occupational Therapist position is based in the Wake Island and Honolulu area, serving clients in their homes and within Assisted Living Facilities. As part of a growing senior care organization, you'll focus on home health, fall prevention, and restoring confidence in everyday movement. Reliable transportation is required, since you'll be traveling to clients across the community.

Key Responsibilities

• Evaluate each client's physical or occupational therapy needs and design individualized treatment plans.
• Provide hands-on, home-based therapy that promotes recovery, strength, and independence.
• Lead fall-prevention strategies and safety assessments for seniors living at home or in Assisted Living Facilities.
• Track progress, adjust care plans, and document outcomes accurately.
• Partner with families, caregivers, and the care team to support each resident's wellbeing.

Qualifications

• Current, valid license as a Physical Therapist or Occupational Therapist in Hawaii.
• Reliable transportation to reach clients throughout the Honolulu area.
• A genuine commitment to senior care, rehabilitation, and home-based health services.
• Strong communication skills and the ability to work independently in varied home settings.
