About the Role
As an RN Case Manager, you'll ensure patients receive seamlessly coordinated, evidence-based care throughout their healthcare journey. Working days (8:00 AM – 4:30 PM) with rotating weekend schedules, you'll combine clinical nursing expertise with strategic care coordination to achieve positive patient outcomes and facilitate smooth transitions across care settings.
What You'll Do
- Assess and manage patient cases to develop individualized care plans aligned with clinical and social needs
- Coordinate discharge planning and arrange continuity of care to prevent readmissions
- Serve as the central liaison between patients, families, clinicians, and community support resources
- Track patient progress, evaluate care effectiveness, and adjust interventions as clinically indicated
- Partner with interdisciplinary teams—physicians, social workers, therapists, and specialists—to optimize safety and outcomes
- Connect patients with available community services, insurance resources, and long-term support options
What We're Looking For
- Current RN license in Florida or eligibility to obtain licensure
- Demonstrable clinical judgment and sound decision-making in complex patient situations
- Excellent oral and written communication skills
- Prior experience in care coordination, case management, or related healthcare roles
