About This Position
You'll serve as a Registered Nurse Case Manager in a full-time, dedicated-placement role beginning January 5, 2025 through April 5, 2025. The position features a consistent schedule of five 8-hour shifts per week (40 hours total), with no floating between multiple units. Based in the Los Angeles area, this assignment allows you to focus on in-depth patient relationships and coordinated care delivery within a single care setting at our healthcare client.
Key Responsibilities
- Conduct patient assessments and develop individualized care plans aligned with clinical goals
- Coordinate care across healthcare teams and external services to optimize patient outcomes
- Advocate for patient needs and facilitate communication between patients, families, and providers
- Monitor treatment adherence and adjust plans as patients' clinical circumstances change
- Maintain comprehensive, accurate documentation in patient medical records
- Support discharge planning and connection to community-based resources
Who We're Looking For
- Active Registered Nurse (RN) license in the State of California
- Proven experience in case management, care coordination, or related nursing specialties
- Excellent written and verbal communication abilities
- Capacity to work collaboratively across interdisciplinary care teams
- Strong organizational and documentation skills with attention to detail
