Position Summary

As a Speech-Language Pathologist supporting our home health program in Oakland County, Michigan, you'll work field-based across diverse patient settings, conducting thorough evaluations and delivering individualized treatment within skilled home health scope. Your role centers on patient independence: restoring communication and swallowing safety, addressing cognitive-communication needs, and building caregiver competence so the entire family becomes part of the recovery plan. Your Responsibilities

  • Conduct comprehensive speech-language evaluations in home settings and deliver skilled treatment as outlined in physician orders and individualized treatment plans.
  • Assess and treat communication disorders (including aphasia), cognitive-communication impairments (attention, memory, executive function), and dysphagia screening and swallow education within home health scope and medical necessity.
  • Develop patient-centered care plans with measurable, functionally meaningful goals that align with each patient's and family's priorities and the care team's objectives.
  • Coordinate across the multidisciplinary team—nurses, physicians, social workers, therapists—communicating clinical observations, plan updates, and any changes in patient status.
  • Provide hands-on coaching to patients and caregivers using evidence-based teach-back and carryover strategies, embedding therapeutic techniques into daily routines and home life.
  • Maintain thorough, timely documentation that meets home health compliance, clinical defensibility, and operational efficiency standards.

What We're Looking For

  • Active Michigan Speech-Language Pathology license in good standing.
  • Strong clinical reasoning and demonstrated capacity to work independently in home-based practice with minimal remote supervision.
