About the Role
As a mental health professional with Ultra Health, you'll deliver therapeutic services and behavioral health support to older adults across our senior care settings. You'll conduct mental health assessments, facilitate individual and group counseling, develop personalized treatment plans, and collaborate with nursing and medical staff to address the full spectrum of mental health needs in aging populations—from depression and anxiety to grief and cognitive concerns. Your clinical expertise will help enhance residents' emotional resilience, social engagement, and overall wellness.
What You'll Do
- Perform comprehensive psychosocial and mental health assessments for residents
- Provide evidence-based individual therapy and counseling sessions
- Facilitate therapeutic support groups addressing life transitions, loss, and emotional health
- Develop and implement resident-centered treatment plans with interdisciplinary care teams
- Maintain thorough clinical documentation and progress notes in compliance with regulations
- Coordinate care with family members, external providers, and facility staff
- Participate in care conferences and contribute clinical insights to team meetings
- Support staff education on geriatric mental health and behavioral best practices
What We're Looking For
- Valid state licensure as LCSW, LPC, or LMFT in Tennessee
- Experience with older adults or long-term care mental health services (preferred)
- Knowledge of age-appropriate therapeutic approaches and geriatric mental health
- Strong clinical assessment, treatment planning, and documentation abilities
- Excellent communication skills with residents, families, and care teams
- Genuine commitment to person-centered, mission-driven care and resident dignity
- Ability to thrive in a collaborative, interdisciplinary healthcare environment
