About the Role

Reporting to the Clinical Manager, you'll be the primary healthcare provider for a diverse patient population receiving skilled nursing services at home. This position offers the rewarding opportunity to build relationships with patients and families while delivering evidence-based care tailored to individual recovery goals and health needs.

What You'll Do

• Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the clinical team
• Perform skilled nursing interventions including medication management, wound care, catheter care, and other clinical treatments
• Monitor patient progress and vital signs, documenting findings and communicating changes to physicians and care coordinators
• Educate patients and families about medications, treatment plans, disease management, and self-care strategies
• Coordinate care with physicians, specialists, social workers, and other healthcare team members
• Maintain accurate, timely documentation in compliance with regulatory requirements
• Respond to patient concerns and provide advocacy for quality care and safety

What We're Looking For

• Valid Registered Nurse (RN) license in good standing in your state
• Clinical nursing experience, preferably in home health, acute care, or community health settings
• Strong assessment skills and ability to think critically in independent patient situations
• Excellent communication and interpersonal skills with patients, families, and care team members
• Reliable transportation and flexibility to travel to patient homes throughout the service area
• Commitment to patient-centered care and maintaining professional nursing standards
• Proficiency with electronic health records and documentation systems
