About the Role
As a Graduate Practical Nurse at Belen Meadows HC and Rehab Ctr, you will provide direct patient care in our skilled nursing facility while supporting the licensed nursing team. This is a 12-hour shift position offering both day and night availability, with flexibility to grow your skills in a structured healthcare environment.
What You'll Do
- Deliver compassionate, evidence-based nursing care to patients in our rehabilitation and long-term care settings
- Assist with activities of daily living, hygiene, and comfort measures
- Monitor vital signs and report changes in patient condition to supervising nurses
- Support medication administration and wound care under licensed nurse supervision
- Maintain accurate patient records and documentation
- Collaborate with interdisciplinary care teams to support patient recovery and wellness goals
What We're Looking For
- Current or recent graduate from a Licensed Practical Nurse (LPN) program
- Valid state nursing licensure or eligible for licensure by examination
- Strong communication and patient care skills
- Ability to work 12-hour shifts and adapt to day or night schedules
- Commitment to professional nursing practice and patient advocacy
