About the Role
You'll serve as a trusted advocate and educator for families navigating early childhood. What You'll Do
- Conduct regular home visits to assess family strengths, challenges, and developmental needs
- Provide guidance on child development, parenting strategies, and family wellbeing
- Connect families to community resources, health services, and educational programs
- Observe and document child progress using evidence-based screening tools
- Coordinate with health professionals, educators, and social services on behalf of families
- Respond to crisis situations with empathy and appropriate referrals
- Maintain detailed case notes and program documentation
- Collaborate with your team to ensure families receive comprehensive support
