Core Duties
- Provide direct assistance with activities of daily living and personal care
- Support clients with community outings, appointments, and transportation
- Help clients develop and practice life skills that increase independence and confidence
- Maintain accurate, timely documentation of client progress and daily activities
- Communicate effectively with the interdisciplinary care team to coordinate support and share updates
- Respond to client needs with patience, empathy, and professionalism
- Encourage meaningful social connections and participation in recreational activities

- Clear communication skills and the ability to listen actively to clients and colleagues
