Your Responsibilities

• Create, launch, and continuously improve clinical pharmacy programs focused on medication optimization and patient safety.
• Complete in-depth medication therapy reviews and deliver actionable, evidence-based recommendations to providers and patients.
• Partner with healthcare colleagues to embed clinical pharmacy input into comprehensive care approaches.
• Track key program indicators, generate clear performance summaries, and recommend data-driven enhancements.
• Deliver targeted education to patients, caregivers, and clinical staff on safe medication use and adherence.
• Maintain full compliance with pharmacy regulations, professional standards, and quality requirements in every activity.

Skills & Qualifications

• PharmD degree from an accredited school of pharmacy plus current, unrestricted pharmacist licensure.
• Background in clinical pharmacy practice, medication therapy management, or healthcare program design.
• Proven analytical skills for assessing clinical data and measuring program results.
• Clear written and verbal communication abilities suited to remote collaboration with teams and individuals throughout the United States of America.
• Comfort using electronic health record platforms, pharmacy systems, and standard productivity tools.
• Self-directed work style that thrives in a full-time remote setting while managing multiple priorities.
