Role Overview

We are seeking a dedicated Case Manager to join our team in the Somersby, Gosford area. As a Case Manager, you will play a crucial role in supporting children and young people within our community by working closely with families, carers, and communities to ensure their needs are met and they can thrive.

Responsibilities

• Provide case management services to children and young people in the community
• Work collaboratively with families, carers, and other professionals to develop and implement support plans
• Conduct assessments and evaluations to identify needs and provide appropriate interventions
• Maintain accurate records and documentation of services provided
• Ensure compliance with relevant policies and procedures

What You Bring

• A degree in Social Work or a related field (or equivalent experience)
• Demonstrated experience in case management, preferably within the child welfare sector
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassion and empathy towards children and families
