The Opportunity

Moments Hospice is seeking a dedicated Registered Nurse (RN) Case Manager to join our compassionate and supportive team. As an RN Case Manager, you will play a crucial role in providing comprehensive care coordination for hospice patients and their families.

Core Duties

The RN Case Manager will be responsible for:

  • Participating in team meetings and case conferences

Requirements

To be considered for this position, candidates must:

  • Hold a current RN license in the state of Minnesota
  • Possess a minimum of 1-3 years of experience as an RN in hospice or home health care
  • Show strong communication and interpersonal skills
  • Have excellent organizational and time management abilities
  • Be able to work independently and as part of a team
  • Possess a valid driver’s license, with the ability to use a fleet car provided by Moments Hospice for both business and personal use at minimal cost
