The Opportunity
Moments Hospice is seeking a dedicated Registered Nurse (RN) Case Manager to join our compassionate and supportive team. As an RN Case Manager, you will play a crucial role in providing comprehensive care coordination for hospice patients and their families.
- Provide patient-centered care through case management services
- Collaborate with interdisciplinary teams to ensure holistic patient care
- Coordinate discharge planning and community resources
- Evaluate and monitor the effectiveness of care plans
- Ensure compliance with regulatory standards and policies
Core Duties
The RN Case Manager will be responsible for:
- Assessing patient needs and developing individualized care plans
- Facilitating communication between patients, families, and healthcare providers
- Managing patient records and maintaining accurate documentation
- Providing education to patients and their families on hospice services and care options
- Participating in team meetings and case conferences
Requirements
To be considered for this position, candidates must:
- Hold a current RN license in the state of Minnesota
- Possess a minimum of 1-3 years of experience as an RN in hospice or home health care
- Show strong communication and interpersonal skills
- Have excellent organizational and time management abilities
- Be able to work independently and as part of a team
- Possess a valid driver’s license, with the ability to use a fleet car provided by Moments Hospice for both business and personal use at minimal cost
