About the Role

As a Care Manager RN, you'll play a central role in coordinating comprehensive care across our interdisciplinary team. Working 8-hour day shifts in a part-time capacity, you'll collaborate with physicians, specialists, and support staff to ensure patients receive integrated, evidence-based care that aligns with their health goals and personal values.

What You'll Do

• Assess patient conditions and develop individualized care plans in partnership with the clinical team
• Coordinate services and resources to support seamless transitions in patient care

What We're Looking For

• Current RN license in California
• Clinical nursing experience demonstrating patient assessment and care coordination skills
