About the Role

This full-time evening shift LPN position provides direct patient care in our long-term care facility serving seniors with complex medical and social needs. You'll work 40 hours per week on evening shifts (8-hour shifts), contributing to continuity of care during a critical time when residents need dedicated nursing support. This LPN role bridges clinical excellence with resident well-being in a long-term care setting.

What You'll Do

  • Administer medications and monitor residents' responses to treatments under the supervision of the registered nurse and physician team
  • Assess and document resident health status, vital signs, and symptoms throughout each evening shift
  • Provide direct patient care including wound care, catheter care, and assistance with activities of daily living
  • Maintain detailed, accurate medical records and communicate patient updates to the nursing team
  • Support residents' comfort, dignity, and quality of life during evening and night hours
  • Collaborate with nursing staff, physicians, and interdisciplinary team to optimize resident outcomes

What We're Looking For

  • Current Minnesota LPN license in good standing
  • Demonstrated experience in long-term care or senior care settings
  • Strong clinical skills in medication administration, vital signs monitoring, and patient assessment
  • Excellent communication and teamwork abilities

  • Ability to work independently during evening shifts while maintaining professional standards
