Social Worker – Remote/Hybrid Central PA., Lehigh County territory

Join Highmark Health's expanding care delivery team as a Remote/Hybrid Social Worker serving Central Pennsylvania and the Lehigh County region. This is a full-time position based out of Penbrook, Pennsylvania, where you'll deliver compassionate psychosocial support to patients navigating complex medical and life transitions.

About the Role

ECCM, the region's leading palliative care service, is seeking a dedicated social worker to support patients and families through every stage of their care journey. Working both in-home and virtually, you'll address the emotional, social, and practical challenges that accompany serious illness. Your work directly improves patient outcomes by connecting families with essential resources, facilitating difficult conversations, and ensuring comprehensive, person-centered support.

What You'll Do

• Conduct psychosocial assessments to identify complexity and determine appropriate interventions for each patient
• Provide crisis support and care coordination for patients experiencing emotional distress or social barriers
• Connect patients and families with community resources, financial assistance programs, and supportive services
• Collaborate with interdisciplinary care teams to integrate social and emotional needs into comprehensive treatment plans
• Deliver supportive counseling and evidence-based coping strategies during transitions in care status
• Manage hybrid workflow, conducting both home-based visits and telehealth appointments across Central PA and Lehigh County

What We're Looking For

• Current LCSW, LSW, or equivalent social work licensure (or actively pursuing)
• Experience in medical social work, palliative or hospice care, or community health settings
• Strong interpersonal and communication skills, including comfort with end-of-life conversations
• Ability to work independently while collaborating effectively within interdisciplinary teams
• Comfort balancing in-home visits with virtual appointments and remote administrative work
• Compassion and emotional resilience when supporting patients and families through serious health challenges
