Home Health Physical Therapist (PT)

Lifespan Therapy is hiring a Home Health Physical Therapist (PT) in Manhattan Beach, Los Angeles County, to deliver skilled rehabilitation directly to patients in their homes. Position Summary

In this role, you'll provide direct physical therapy services to medically complex, homebound patients throughout our service area. You'll conduct thorough in-home assessments, design individualized treatment plans, and guide patients through exercises and mobility training to help them regain independence and quality of life. This is an ideal position for PTs seeking to transition from outpatient or skilled nursing facility settings or for experienced home health clinicians looking to expand their practice with an organization that values clinical expertise and work-life balance.

Day-to-Day Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes, assessing mobility, strength, balance, and functional capacity
• Develop and implement evidence-based treatment plans tailored to each patient's goals and medical history
• Conduct therapeutic exercises, manual therapy, and functional training to improve mobility and prevent falls or complications
• Educate patients and caregivers on home exercise programs, safety modifications, and lifestyle strategies
• Maintain detailed clinical documentation and communicate progress to physicians and interdisciplinary care teams
• Monitor patient outcomes and adjust treatment protocols based on functional improvements and medical changes

Requirements

• Current California Physical Therapy license (or ability to obtain licensure)
• Home health PT experience preferred, or recent transition from outpatient or skilled nursing facility practice
• Strong clinical assessment and manual therapy skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license (required for in-home patient visits)
• CPR certification (BLS preferred)
